How to Create and Manage Forms

With a form, you can easily collect specific information from your employees.

Question Type

1. Text Question

Collect textual input

- Is Text Area: Expands for longer responses.
- Max Characters: Set a character limit.

- Short responses like remarks, or general comments.

2. Numeric Question

Gather numbers or ratings

- Min/Max Values: Set a numeric range.
- Multiple: Set increments (e.g., multiples of 5).

- Simple Numeric: “How many hours do you work per week?”
- Scale Numeric: “Rate your satisfaction 1-10.”

3. Single Selection Question

Select one option from a list

- If more than 5 options, switches to dropdown.
- Add Options: Enter options or click the + button.

- Choose one option (e.g., “Which department are you in?”).

4. Multi-Selection Question

Select multiple options from a list

- Add Options: Same as single selection.

- Select multiple preferences (e.g., “Which benefits would you like to receive?”).

5. Yes/No Question

Binary choice (Yes or No)

- Translated to the respondent's preferred language automatically.

- “Do you agree with the new company policy?”
- “Are you available to work overtime this week?”

6. Date Question

Collect a specific date

- Min/Max Date Range: Set a date range.

- “What is your preferred start date for the new project?”
- “Please provide your date of birth.”

7. Image/File Upload Question

Allow file or image upload

- Respondents can upload from their device or take a new picture using their camera.
- Note: Cannot ask for predefined image selection (e.g., logo).

- Upload a picture or document (e.g., “Submit your ID card” or “Upload your resume”).

To swap questions between pages or reorder them:

  1. Click the 3-line (burger) icon next to the question you want to move.

  2. Drag the question to a new position on the page or to a different page.

This feature helps you customize your survey structure effortlessly.

Example Form:

An example form might include a Yes/No question about whether the employee is requesting time off, followed by a text field to explain the reason, and a date field to choose the requested days off.

Tips for Creating Effective Forms:

  • Keep Questions Clear & Concise: Ensure that your questions are straightforward and easy to understand.

  • Use the Right Question Types: Choose the appropriate question type based on the kind of information you want to collect (e.g., text, numbers, yes/no, etc.).

  • Limit the Number of Options: For single and multi-selection questions, keep your list of options manageable. If you have more than five options, consider using a dropdown.

  • Test Your Form: Always preview your form before publishing to ensure it looks and functions as expected.
